Positive Impact Health Centers, Inc. seeks a Medical Case Manager to provide comprehensive case management for adult clinic patients within an interdisciplinary team including medical and behavioral health staff.

The role involves adherence counseling, service planning, and coordinating referrals to ensure high-quality health outcomes under HRSA/Ryan White guidelines. Strong communication and confidentiality are essential.

Qualifications

• Social work degree or nursing/case management background expected.
• Experience with HIV care frameworks and community health settings preferred.
• Fluency in Spanish is highly preferred.

Responsibilities

• Learn and apply HIV/AIDS care guidelines and case management practices.
• Provide case management to clinic patients, develop ISP and monitor progress.
• Coordinate referrals for medical and non‑medical services and assist with eligibility for medication programs.
• Document patient information accurately in EMR and maintain HIPAA compliance.
