Job summary

The Health Federation of Philadelphia is seeking a Medical Case Manager to provide practical and emotional support to HIV/AIDS-affected clients in Philadelphia. You will help clients manage daily living activities, maintain medical stability, and ensure adherence to treatment plans under clinical guidance.

This role involves coordinating care with health systems, linking clients to services, and maintaining confidentiality.

Qualifications

• 2+ years direct service experience in a human service, health care, community, or educational setting.
• Experience with maternal, pediatric, and family health issues and insurance/medical assistance procedures with economically disadvantaged is preferred.
• Bi-lingual in English/Spanish or English/French is preferred.

Responsibilities

• Conduct initial pre-screening of patients to determine eligibility for services.
• Develop a comprehensive care plan with the patient focusing on short and long term health goals.
• Establish and implement a medical treatment plan with counseling on medications and appointments.
• Identify barriers to care and coordinate with Health System Navigators to remove them.
• Annual reassessment of the client care plan based on health changes.
• Provide referral services to promote adherence and access to care.
• Monitor follow-through with services and evaluate outcomes.
• Attend workshops and staff meetings to maintain licensure and knowledge.
• Complete special projects as directed by supervisor.
