Job summary

The Wright Center for Graduate Medical Education seeks a Ryan White Part C Medical Case Manager and Eligibility Coordinator to manage medical service coordination for individuals living with HIV/AIDS and assess eligibility for Part C.

This role supports access to healthcare and services, conducts case management, maintains documentation, and advocates for patients while collaborating with clinicians and community partners.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field or equivalent experience.
• Knowledge of HIV is preferred.
• BLS certification is required.
• Excellent verbal and written communication skills.
• Demonstrated cultural competence with diverse populations, esp. in HIV/AIDS.

Responsibilities

• Conduct comprehensive assessments of clients' medical, social, and support needs.
• Determine eligibility for the Ryan White Part C program.
• Recertify Ryan White Part C patients every six months for income, residency, insurance, and ID.
• Maintain documentation of determinations and update records.
• Coordinate medical appointments and ensure follow-up care.
• Advocate for patients to access treatments and services.
• Enter notes within five business days of care activities.
• Attend interdisciplinary team meetings weekly and CM huddle.
