Positive Impact Health Centers, Inc. is seeking a Medical Case Manager to provide comprehensive case management to adult clinic patients, coordinating medical and social services to support health outcomes.

You will work within an interdisciplinary team, following HRSA/Ryan White guidelines and maintaining HIPAA compliance. The role involves developing and monitoring individualized service plans, navigating medication access programs, and documenting all activities in the EMR.

Qualifications

• Ability to perform biopsychosocial assessments and develop an ISP.
• Effective communication across diverse populations and teams.
• Knowledge of HIV medical and case management fundamentals.
• Ability to work independently and manage time effectively.
• Spanish language proficiency preferred.

Responsibilities

• Learn and stay current on HIV/AIDS and case management practices.
• Provide case management to identified clinic patients per HRSA/Ryan White guidelines.
• Complete biopsychosocial assessments and create ISP.
• Manage an average caseload of 70 patients and monitor ISP progress.
• Coordinate referrals for medical and non-medical services; document in EMR.
• Assist with applications for medication access programs (ADAP).
• Abide by HIPAA and agency policies.
