Job summary

AbsoluteCare is seeking a Patient Navigator to support linkage to care, engagement, and retention across HIV, PrEP, HCV, and harm reduction services. Qualifications

• High School diploma or equivalent required.
• Experience in HIV, harm reduction or related public health fields preferred.
• Ability to communicate effectively with diverse populations and care teams.

Responsibilities

• Provide in-person and virtual navigation, follow-up, and support to members across HIV care, PrEP/PEP, hepatitis, and harm reduction services.
• Deliver status-neutral navigation across prevention and treatment pathways.
• Ensure timely linkage to care and ongoing retention support with a caseload of high-priority members.
• Coordinate with providers, clinics, and care teams to schedule appointments and address disengagement.
• Maintain accurate documentation in the EMR and ensure confidentiality.
