Job summary

COLORADO HEALTH NETWORK INC is seeking a Clinical Patient Navigator to support PrEP and HIV/HCV programs at the Denver clinic. The role involves eligibility assessments, insurance assistance, health education, and coordinating referrals to medical and social services.

Under the Clinical Services Officer, you will manage an active caseload, provide outreach and monitoring, and maintain accurate EHR documentation. Bilingual preferred; evenings/weekends as needed.

Qualifications

• Assess eligibility and provide insurance assistance to clients.
• Educate clients about HIV/HCV health education and PrEP navigation.
• Maintain active caseload and coordinate referrals to care services.

Responsibilities

• Provide screenings and outreach to promote PrEP and HIV/HCV programs.
• Coordinate and maintain an active caseload of PrEP and HIV/HCV patients.
• Assist with access to medications and adherence support.
• Enter case notes and maintain timely data entry in the EHR.
• Serve as liaison between patients and care teams.
