Job summary
A health center dedicated to community support is seeking a HPAC Patient Navigator to conduct outreach and connect low-income individuals with HIV prevention services and primary care. The successful candidate will work closely with diverse populations, assist clients in accessing healthcare, and conduct health education activities. A high school diploma, relevant experience, and fluency in Spanish or Mandarin are highly preferred. The role includes flexible hours and teamwork in a supportive environment.
Qualifications
High school diploma/GED with 2 years of working experience required.
Energetic and motivated to achieve the agency mission.
Ability to recruit from target population required.
Ability to work flexible hours including some evenings and weekends.
Ability to provide HIV testing within 30 days of starting work.
Responsibilities
Conduct outreach to recruit low-income, high-risk individuals.
Identify undiagnosed HIV persons and link them to healthcare.
Link diagnosed individuals in care to maximize virus suppression.
Assist clients with access to primary care and specialty services.
Conduct health education activities and screenings.
Skills
Fluency in Spanish or Mandarin
Strong socio-cultural identification with the target population
Ability to work with diverse populations
Ability to handle multiple tasks
