Job summary

Qualifications

• Bachelor's degree in a health-related field is required.
• One year of experience working with the HIV population is required.
• Minimum of 5 years proven work experience as a team leader is required.
• Knowledge of performance metrics such as PDSA cycles is required.

Responsibilities

• Develops the clinic teams' goals based on PCMH requirements and effective communication.
• Responsible for the overall coordination of PCMH sessions and ensures patients are linked to care and obtain viral load suppression.
• Facilitates effective communication between patients, team members, disciplines, and administrators.
• Monitors team and clinic performance using data metrics such as HEDIS measures and participates in CQI Committee continuous quality improvement efforts.
• Encourages staff development and team building efforts.
• Oversees the teams' coordination of internal and external referrals and monitors processes.
• Adheres to Department's Documentation Policy of all Patient Encounters and Forms.
• Enhances Professional Development and maintains an area of specialization.
• Participates in all CORE Center and department mandatory trainings and meetings.
• Provides excellent customer service and professional interactions with clients and internal/external customers.
