Job summary

Apicha Community Health Center in Manhattan, NY seeks a Care Coordination Team Leader to oversee psychosocial support for clients living with HIV, lead the Be Into Health program, and supervise Care Coordinators. This is an on-site role requiring strong clinical leadership and program management.

Responsibilities include developing care plans, conducting assessments, coordinating services, ensuring accurate documentation, and promoting quality assurance.

Qualifications

• Advanced degree in mental health counselling or social work (with 2+ years experience).
• Valid New York State license to provide mental health services (LMSW or equivalent).
• Experience supervising paid staff is a plus.
• Experience with HIV/AIDS clients and SMI/SUDs is valued.
• Commitment to Apicha's mission and values.
• MS Office skills; familiarity with AIRS/eSHARE/eCW preferred.

Responsibilities

• Provide day-to-day oversight of Be Into Health program and ensure service delivery per policy.
• Supervise Care Coordinator and participate in performance reviews.
• Develop and monitor program deliverables and reports.
• Conduct social network assessments and develop/update client care plans.
• Create curriculum for psychosocial group and individual counseling.
