Job summary

Cook County Health & Hospitals System in Chicago is seeking a bilingual Case Manager for the CCHIP program to link clients with health care, clinical psychosocial, and supportive services for HIV/AIDS patients. The CM will assist with treatment adherence, connect clients to primary medical services, and coordinate benefits enrollment.

Requires Spanish bilingualism, two years of related experience, and a relevant degree; 9:00 AM–5:00 PM, grant-funded with potential renewal.

Qualifications

• Minimum qualifications include an associate's or bachelor's degree in a related field.
• Bilingual Spanish is required.

Responsibilities

• Complete intake and assess client needs.
• Provide referrals to behavioral health, education, dental, and benefits.
• Maintain caseload per funder requirements.
• Develop individualized service plans and monitor progress.
• Coordinate with CORE Center and AFC for certifications and trainings.
