Job summary

Public Health Management Corporation (PHMC) in Philadelphia seeks an HIV Care Manager to oversee performance, staffing, and funding for HIV Care across PHMC Health. This role requires deep knowledge of Ryan White programs and coordination with city, state, and funders to ensure access to services.

You will lead regular interdisciplinary meetings, supervise HIV care teams, ensure documentation in EMR and CAREWare, monitor performance measures, and liaise with external partners to optimize program

Qualifications

• 2 years of case management and/or care coordination experience required.
• 2 years of supervisory experience preferred.
• Master's degree from an accredited university or Bachelor's degree in social work, psychology, sociology, or other related field with 3 years of infectious diseases, HIV medical case management, PrEP, and/or care coordination experience is required.

Responsibilities

• Ensure successful execution of Ryan White program requirements and maintain performance on Ryan White measures.
• Provide leadership through interdisciplinary meetings with staff engaged in Ryan White-funded activities.
• Serve as subject matter expert and spokesperson for HIV-related workflows and funders.
• Ensure compliance with DHH and HRSA standards related to HIV treatment and care coordination.
• Monitor and communicate performance data to the health care team.
