Nationwide Children9s Hospital Inc is seeking a non-medical case manager to coordinate access to medical, social and supportive services for individuals living with HIV/AIDS within the Ohio program, focusing on retention in care and viral suppression.

The role involves daily telephone and electronic communications, assessment, and linkage to resources, with opportunities for professional learning and collaboration in a multidisciplinary team.

Competenze

• High School diploma is required.
• Background in health care, public health, or related clinical field preferred.
• Ability to communicate (verbal and written) clearly with professionalism and tact.
• Computer literate with working knowledge of Microsoft products (Outlook, Word, PowerPoint, Excel, Teams).
• Ability to work within a multidiscipline matrix and relate to diverse age and demographic backgrounds.
• Basic knowledge of healthcare terminology.
• Experience working in healthcare in a patient facing role, preferred.

Mansioni

• Initiates telephone contact and completes a health risk assessment. Escalates complex or urgent needs to a supervisor.
• Connects patients to the right program once needs are identified. Develops individualized self-management skills to patients and families through education and resource provision.
• Provides organization of electronic communication related to survey results of the health risk assessment. Documents within the electronic medical record.
• Participates in orientation and continuing education of staff and students as appropriate.
