Job summary

Equitas Health is hiring a Non-Medical Case Manager in Ohio to provide client-centered case management for people living with HIV. You will help with insurance, enrollment, housing stability, and income supports, while coordinating with healthcare providers and community programs.

The role emphasizes NASW-aligned standards, confidentiality, and timely documentation. The position includes benefits such as PTO, health, and retirement options, and union representation.

Qualifications

• Bachelor’s degree in a social science field (Social Work preferred) and at least 6 months experience in a social service setting.
• Associate’s degree with 1-3 years of experience working in a social service setting; OR
• High School Diploma/GED with 4-6 years of experience working in a social service setting

Responsibilities

• Provide high quality case management for clients who are living with HIV and their families with a focus on viral suppression and other improved HIV related health outcomes.
• Complete Ryan White and other assessments, annually and as needed, that identify individual treatment strengths and struggles related to viral suppression, maintenance of income and insurance benefits, and sustainable housing.
• Develop, monitor, and evaluate individual case plans for each assigned client at intake, bi-annually, and as needed thereafter.
• Function as a central and primary access point for financial assistance programs, including but not limited to Ryan White Treatment Modernization Act (Parts A, B and C), HOPWA short term rental assistance, and other programs.
• Identify mental health crises and consult with Supervisors when a crisis occurs; coordinate with Mental Health and Therapy Providers as appropriate.
• Empower clients to link with housing, workforce development, transportation and other supports that promote graduation.
• Engage medical providers to understand HIV-related health factors and referrals to services.
• Represent Equitas Health to educate and advocate for the needs of HIV+ individuals.
• Maintain documentation within Equitas Health and other systems within 48 hours of service.
