Job summary

Equitas Health in Ohio is seeking a Non-Medical Case Manager to provide comprehensive case management for HIV+ clients across Ohio. The role focuses on maintaining insurance benefits, housing stability, and income sources while promoting client independence and health outcomes.

The position is represented by the relevant unions and offers benefits such as PTO, health, 401k, and paid holidays. Strong collaboration with NASW standards and HIPAA compliance is expected.

Qualifications

• Bachelor’s degree in a social science field (Social Work preferred) and at least 6 months experience in a social service setting, preferably 1 or more years; OR
• Associate degree with 1-3 years in social service; OR
• High School Diploma/GED with 4-6 years in social service

Responsibilities

• Provide high quality case management for clients living with HIV and families, focusing on viral suppression and improved health outcomes.
• Complete Ryan White and other assessments to identify strengths and challenges.
• Develop, monitor, and evaluate individual case plans for each client at intake and review points.
• Coordinate financial assistance programs including Ryan White, HOPWA, and related supports.
• Identify mental health crises and consult with supervisors; involve mental health providers as needed.
• Represent Equitas Health in the community to provide education and advocacy.
