Job summary

Old Dominion University Research Foundation seeks an experienced case management professional to support the Center for Comprehensive Care of Immune Deficiency (C3lD). The role involves assisting patients with enrollment in ACA plans, navigating government entitlements, and maintaining accurate demographic and service data.

The position requires a Bachelor’s degree in a relevant field and at least three years of related experience, with preference for Ryan White/HRSA program exposure and grant

Qualifications

• Bachelor’s degree in a relevant discipline and three years of relevant experience in human services.
• Five years’ experience preferred.
• Ryan White Program and/or HRSA Management experience strongly preferred.
• Detail oriented and precise record-keeping.

Responsibilities

• Evaluate and assist patients in overcoming barriers to proper treatment and management of their HIV disease.
• Follow Virginia HIV/AIDS Case Management Guidelines and provide minimum standard of care.
• Provide follow-up and occasional home visits when required.
• Link patients to government entitlements (Social Security Disability, Medicaid, Medicare, Ryan White, ADAP, ACA) and assist with applications.
• Help patients navigate the continuum of care and advocate for themselves.
• Collect and maintain financial and support documentation to support eligibility.
• Coordinate access to financial support services through ongoing assessment of client needs.
• Attend modules, conferences, and meetings to maintain competencies.
• Enter patient demographics into CARE ware, maintain monthly encounters and prepare reports.
• Orient patients to services at C3lD and ensure timely documentation of encounters.
• Assist with ACA enrollment services; ensure culturally and linguistically appropriate service delivery.
• Screen referred patients for eligibility for entitlements.
