Job summary

The Wright Center for Community Health and Graduate Medical Education seeks a Ryan White Part C Medical Case Manager and Eligibility Coordinator to coordinate medical services for individuals living with HIV/AIDS under the Ryan White Part C program. This role also serves as the Eligibility Coordinator, assessing and determining program eligibility.

Reporting to the RW Grants Administrator, the position requires strong communication, advocacy, and documentation skills.

Qualifications

• Strong communication skills, both verbal and written.
• Demonstrated cultural competence and sensitivity when working with diverse populations.
• Ability to advocate for clients to access medical care and social services.
• Solid organization and meticulous documentation skills.

Responsibilities

• Conduct comprehensive assessments of clients' medical, social, and support service needs.
• Determine eligibility for the Ryan White Part C program and recertify as required.
• Maintain documentation of eligibility determinations and update records.
• Coordinate medical appointments and follow‑up care to ensure continuity of healthcare.
• Advocate for clients to access treatments, medications, and support services.
