Job summary

Virginia Commonwealth University (VCU) School of Medicine in Richmond seeks a Ryan White Case Manager to coordinate and assess services for HIV care, ensuring access and retention. You will perform needs assessments, manage care plans, and maintain detailed documentation within clinical and administrative standards.

The role requires 3 years of case management experience, knowledge of social programs and HIPAA, plus strong communication and time-management skills.

Qualifications

• 3 years of Case Management experience in a healthcare delivery system.
• Knowledge of Medicare, Medicaid, and other state or federally funded assistance programs for eligible individuals
• Knowledge of HIPAA and ability to implement privacy and confidentiality practices
• Excellent customer service and interpersonal skills
• Excellent time management and organizational skills
• Ability to demonstrate superior teamwork behaviors
• Ability to work independently
• Proficient in MS Office and data entry programs
• Sensitive to confidential health information and HIV/AIDS related issues and controversies
• Outstanding communication skills, oral and written

Responsibilities

• Screen for initial eligibility for the Ryan White grant program and annual renewals.
• Provide direct social work/case management services to clients living with HIV/AIDS and their families in accordance with their medical diagnosis, psychosocial, social, and financial needs.
• Formulate a plan of care with multidisciplinary teams to promote wellness.
• Complete and maintain documentation of all interactions with client/family.
• Respond to referrals and assist with the referral process.
• Enter CAREWare data in a timely manner and input into EPIC as required.
• Participate in Quality Management activities.
• Attend supervisor-required meetings and trainings.
• Attend one community event annually.
• Complete professional development activities per policy.
