Job summary

The Centers in Cleveland is seeking an experienced Ryan White Medical Case Manager to support clients enrolled in the HIV Ryan White Program. You will work with a multidisciplinary team to assess needs, develop individualized care plans, and connect clients to medical, behavioral health, housing, and social services.

Key duties include coordinating referrals, tracking progress, and advocating for access to care.

Qualifications

• Bachelor's degree in Social Work and LSW required
• Master's degree in Social Work preferred
• Clinical license in good standing with Ohio
• Valid driver's license and insurance coverage ($100,000/$300,000)
• Two+ years of related work experience
• Willingness to work with HIV+ population

Responsibilities

• Conduct comprehensive assessments of client needs, strengths, and barriers to care.
• Develop, implement, and monitor individualized care plans with clients and healthcare providers.
• Facilitate linkage to and retention in medical care, behavioral health services, harm reduction programs, and social services.
• Provide education and counseling on HIV, HCV, substance use, and integrated care strategies.
• Advocate for clients to ensure access to medical treatment, medication assistance, housing, and other essential services.
• Develop relationships/partnerships with local LGBTQ agencies
• Coordinate and track referrals to ensure timely access to services and follow-up care.
• Collaborate with interdisciplinary teams to support holistic client care.
• Lead the program's quality improvement projects and participate in committees.
• Support occasional evenings (ending at 8 p.m.) and one Saturday per month, flexible scheduling.
