You will manage an active caseload, coordinate referrals, and help patients overcome barriers to accessing medical care, while maintaining HIPAA compliance and coordinating with multidisciplinary teams.

Qualifikationen

• Bachelor's or Master's degree in Social Work or Health/Human Services required.
• 2+ years of case management experience required.
• EMR experience preferred.

Aufgaben

• Provide patient navigation and support through the HIV care service delivery system.
• Coordinate patient scheduling for all providers.
• Coordinate referrals to internal and external agencies after appointments.
• Assist patients in overcoming barriers to access services (insurance, transportation, finances).
• Educate patients about HIV, disease progression, prevention, and treatment options.
• Maintain an active caseload of HIV patients and document progress in the EHR.
