Job summary

CARIN Alliance seeks a Medical Case Manager to provide comprehensive medical and social services to adults living with HIV/AIDS. You will support intake, coordinate clinical acclimation, and help patients navigate treatment and social services with empathy and cultural sensitivity.

Qualifications

• Associate’s degree or at least 2 years of relevant experience.
• Experience with HIV services, outreach, counseling, and care management.
• Understanding of substance abuse issues and ability to work within diverse community networks.
• Proficiency with EMRs/EHRs and strong documentation skills.
• Ability to work with clients from various cultural backgrounds; Haitian Creole speaking is highly desirable.

Responsibilities

• Offer case management, support new patient intake, and facilitate clinical acclimation and emotional support.
• Promote medication adherence, patient engagement, and collaborate with healthcare team members to ensure optimal care.
• Refer clients to community agencies to address social determinants of health (SDoH).
• Conduct regular patient visits, update care plans, and maintain accurate documentation.
• Manage referrals for medication assistance programs and coordinate short-term health navigation.
• Attend team meetings and perform additional duties as needed.
