Job summary

Positive Impact Health Centers (PIHC) in Georgia is seeking a Medical Case Manager to provide comprehensive case management for adult clinic patients, within an interdisciplinary team including medical providers, behavioral health clinicians, and support staff.

You will manage an average caseload of around 70 patients, develop individualized service plans, and ensure continuous access to medication and supportive services.

Qualifications

• Bachelor's degree in social work or nursing; MSW/BSW preferred.
• Experience conducting biopsychosocial assessments.
• Ability to coordinate services and develop ISP for ~70 patients.
• Spanish language ability highly preferred.

Responsibilities

• Perform biopsychosocial assessments and ISP development.
• Provide medical case management per HRSA/Ryan White guidelines.
• Coordinate referrals and follow-up for medical and non-medical services.
• Document patient information in EMR accurately and timely.
• Maintain HIPAA compliance and confidentiality.
