Job summary

The Wright Center in Scranton, PA seeks a Ryan White Part C Medical Case Manager and Eligibility Coordinator to coordinate medical services for clients living with HIV/AIDS under the Part C program. Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field, or equivalent education/experience.
• Knowledge of HIV preferred.
• BLS certification required.
• Strong communication, organization, and documentation skills.
• Ability to advocate and coordinate care across providers.
• Commitment to confidentiality and ethics.

Responsibilities

• Conduct comprehensive assessments of clients' medical, social, and support service needs.
• Assess eligibility for the Ryan White Part C program.
• Coordinate medical appointments and follow‑up care to ensure continuity of healthcare.
• Maintain documentation and update records for eligibility determinations.
• Prepare program reports on activities, outcomes, and challenges.
• Enter all notes in CAREWare/EMR within required timelines.
• Participate in IDT meetings and weekly huddle.
• Collaborate with external agencies to enhance client support.
