Job summary

Positive Impact Health Centers (PIHC) seeks a Medical Case Manager to provide case management for adults attending HIV care programs. You will work within an interdisciplinary team including providers, behavioral health staff, and patient navigators to coordinate medical and supportive services.

Responsibilities include biopsychosocial assessments, individualized service planning, and monitoring progress and eligibility for medication access programs.

Qualifications

• Strong social work, nursing, or case management skills.
• Excellent interpersonal and cross-cultural communication abilities.
• Proficient computer skills for documentation and data entry.
• Knowledge of HIV care and case management fundamentals preferred.
• Ability to work independently and in a team.
• Fluent Spanish is highly preferred.

Responsibilities

• Perform biopsychosocial assessments for new and returning patients.
• Develop and monitor individualized service plans for patients.
• Coordinate referrals and follow-up for medical and non-medical services.
• Maintain accurate documentation in the EMR system.
• Participate in an interdisciplinary care team to address patient needs.
• Assist patients with applications for medication access programs.
