Job summary

Meharry Community Wellness Center seeks a Medical Case Manager to coordinate comprehensive patient care plans under the supervision of a Clinical Psychologist. You will liaise with EIS Workers, Community Health Workers, medical providers and community agencies to ensure patients remain in HIV care.

The role requires adherence counseling, care coordination, and linkage to core and supportive services, with data entry in CAREWare and timely reporting.

Qualifications

• Master's degree in a health-related field is preferred; or Bachelor's degree with a minimum of two years related work experience will be considered after special evaluation.
• Experience working with Ryan White standards of care or similar healthcare programs is a plus.
• Familiarity with HIV care coordination and community outreach is desirable.

Responsibilities

• Coordinate comprehensive patient care plans under supervision.
• Serve as clinic liaison between patients, medical providers, and community agencies.
• Maintain contact with patients to ensure continuity of HIV care and reengagement as needed.
• Provide adherence counseling, care coordination, and linkage to core and supportive services.
• Enter data in real-time using CAREWare templates and submit required reports.
• Monitor and support patients through transitions (incarceration, hospitalization, discharge).
• Refer to drug treatment and rehabilitation facilities and monitor outcomes.
