Qualifications

• Minimum one year experience at a social services-related agency or related position.
• Experience providing case management and client advocacy.
• Familiarity with Medicaid, ADAP, and patient navigation is preferred.

Responsibilities

• Provides medical case management to persons infected and affected by HIV/AIDS across four-county area.
• Performs comprehensive Needs Assessments and develops individualized care plans.
• Monitors treatment adherence, attendance, and family planning needs.
• Links clients to Medicaid, PAC Waiver, ADAP, PAP, and other entitlements; ensures payer sources are used.
• Maintains accurate documentation in CAREWare and HMS databases within required timelines.
• Coordinates transportation and referrals to community and health services.
