Job summary

The Florida Department of Health seeks a Human Services Analyst to provide medical case management for HIV/AIDS in Indian River, Martin, Okeechobee, and St. Lucie counties, performing assessments, care planning, and ongoing monitoring.

You will coordinate services, ensure payer sources are used appropriately, and document in CAREWare/HMS databases. Responsibilities include conducting acuity assessments, developing individualized care plans, linking to ADAP and other programs, and advocating for

Qualifications

• One year experience at a social services agency or related position.
• Must be able to work before, during and beyond normal hours in emergencies.
• Must be able to communicate effectively in English; bilingual abilities preferred.

Responsibilities

• Provide medical case management for HIV/AIDS in Indian River, Martin, Okeechobee, and St. Lucie counties.
• Complete comprehensive needs assessments and develop individualized care plans.
• Monitor plan implementation, re-evaluate acuity, and ensure continuity of care.
• Linkage and referrals to agencies, payer sources, and community resources.
• Document all client activities in CAREWare and HMS within three business days.
• Provide client navigation, insurance education, and support.
• Participate in meetings and complete required trainings.
