Responsibilities

• Initiates Medical Case Management Activities and provides services within established policies, procedures and time frames to provide care appropriate to patient population.
• Develops an individualized care plan and coordinates services to implement the plan.
• Monitors and reevaluates/updating care plans as needed.
• Provides client-specific advocacy and reviews service utilization.
• Links clients to medical care through outside agencies (e.g., shelters, treatment facilities).
• Engages clients to build trust and communicate available services.
• Analyzes social situation to remove barriers to care and ensures adherence counseling for HIV/AIDS treatments.
• Monitors adherence and follows up to ensure continued engagement in care.
• Maintains linkage to medical care through collaboration with hospital case management and referral agencies.
• Provides community education and information materials; maintains confidentiality per HIPAA and related regulations.
• Attends continuing education and participates in staff training and performance improvement activities.
