Job summary

Monument Health is seeking an HIM Coder II to accurately assign ICD-10, CPT, and HCPCS codes across hospital outpatient and professional services. The coder will apply medical knowledge and coding guidelines to ensure proper billing and compliance.

The role includes maintaining productivity, engaging with physicians for documentation, and resolving payor edits while utilizing encoder and grouper tools. Certification and ongoing training are encouraged to support professional growth.

Qualifications

• Proficient in ICD-10, CPT, HCPCS coding per official guidelines.
• Experience with hospital outpatient and professional coding.
• Familiar with coding guidelines, payor standards and denial resolutions.
• Knowledge of NCCI and LCD/NCD guidelines.

Responsibilities

• Review medical records to abstract data for coding.
• Code hospital and professional services with ICD-10, CPT and HCPCS codes.
• Apply coding guidelines to select correct codes for billing.
• Maintain productivity and accuracy in coding tasks.
• Query providers for missing documentation as needed.
• Address payor edits and denials and communicate with related teams.
• Use encoder and grouper software to code efficiently.
• Support coding education and training within the department.
