Job summary

Apex Home Health, Hospice & Home Care seeks a Charge Nurse (RN) to lead and coordinate care on a high-risk Cardiac Progressive Care Unit. You will guide complex care plans, collaborate with an interprofessional team, and drive evidence-based practice initiatives from admission to discharge.

Qualifications include Nursing Diploma/Associate's with RN license, BLS & ACLS, and 2–5 years of RN experience; a BSN and 5–10 years are preferred.

Role

Responsible for leading and coordinating care on a high-risk Cardiac Progressive Care Unit as a Charge Nurse (RN).

Key Responsibilities

Facilitate patient care from admission through discharge, develop and implement complex care plans, and collaborate with an interprofessional team. Lead evidence-based practice initiatives, evaluate patient and family satisfaction, and ensure safety standards.

Qualifications & Skills

Nursing Diploma/Associate's degree with 2-5 years of experience; RN license required. Preferred candidates will have a Bachelor's degree and 5-10 years of experience. Must hold BLS, ACLS, and extensive EKG certifications. Strong leadership, critical thinking, and communication skills are essential.
