Job summary

JFCS Phoenix is seeking a compassionate High Needs Case Manager to support children in behavioral-health services as they progress toward self-sufficiency and potential family reunification. You will provide home visits, coordinate services with agencies, attend CFT meetings, and ensure paperwork is completed accurately.

Qualifications

  • Requires educational or work experience in behavioral health.
  • High School Diploma or GED with 4 years of behavioral health experience.
  • AA degree with 2 years of behavioral health experience.
  • Bachelor’s Degree in Social Work, Psychology, or human services field.

Responsibilities

  • Provides case management services to children enrolled in the behavioral-health system.
  • Provides weekly contact with assigned clients, including home/community visits and medication checks.
  • Assists youth in navigating discharge from hospitals and finding intensive services as clinically indicated.
  • Coordinates services and collaborates with agencies such as DCS, Probation/Parole, DDD, and schools.
  • Coordinates and facilitates Child & Family Team (CFT) Meetings.
  • Completes referrals, prior authorizations, and health plan paperwork for enrollment.
  • Collaborates with clinical team to ensure Integrated Service Plans and meeting notes are completed accurately.

**Phoenix Location**


High Needs Case Manager: JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services.


Responsibilities


  • Provides case management services to children enrolled in the behavioral-health system.

  • Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.

  • Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.

  • Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.

  • Coordinates and facilitates Child & Family Team (CFT) Meetings.

  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.

  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.


Qualifications

-Meets one of the following education/experience requirements:



  • High School Diploma/GED required and 4 years of Behavioral Health experience.

  • A.A. Degree and two years of Behavioral Health experience

  • Bachelor’s Degree in Social Work, Psychology, or human services field.


-Additional qualifications:



  • Level 1 Fingerprint Clearance Card without Restriction(s)

  • Meet Agency requirements for MVR (Motor Vehicle Record) report

  • Reliable transportation

  • AZ driver’s license

  • AZ auto insurance
