Job summary

Jewish Family and Children’s Service (JFCS) in Phoenix, AZ, seeks a High Needs Case Manager to support children in behavioral-health services on the path to self-sufficiency and possible family reunification. The role includes guiding families, coordinating services, and ensuring timely paperwork while working with a clinical team.

Applicants should have a relevant degree or experience, a valid AZ driver’s license, and reliable transportation to travel for home visits and community settings.

Qualifications

• High School Diploma or GED required with 4 years of behavioral health experience.
• A.A. degree with two years of behavioral health experience also acceptable.
• Bachelor’s in Social Work, Psychology, or human services preferred.
• Level 1 Fingerprint Clearance Card without restrictions.
• Meet agency MVR requirements and have reliable transportation.

Responsibilities

• Provide case management for children in the behavioral-health system.
• Maintain weekly contact with clients, including home visits and medication checks.
• Assist youth with hospital discharge planning and access to intensive services.
• Coordinate services with DCS, probation/parole, schools and other providers.
• Facilitate Child & Family Team (CFT) meetings and complete required paperwork.
• Collaborate with clinical team to finalize Integrated Service Plans and notes.
