Job summary

Jewish Family & Children’s Service of Arizona is seeking a compassionate High Needs Case Manager to support children in behavioral-health services as they strive for self-sufficiency and potential family reunification.

The role involves weekly client contact, home/community visits, discharge planning, and collaboration with DCS, schools, and other providers. Strong documentation and teamwork are essential.

Qualifications

• Experience in behavioral health services and coordination with multiple agencies.
• Ability to conduct home/community visits and discharge planning.
• Strong documentation and paperwork skills for health plans.

Responsibilities

• Provides case management services to children enrolled in the behavioral-health system.
• Maintains weekly contact with clients, including home/community visits and medication checks.
• Assists youth with discharge planning and coordinating intensive services as indicated.
• Coordinates with DCS, Probation/Parole, DDD, schools and other providers.
• Facilitates Child & Family Team (CFT) Meetings and completes required referrals and authorizations.
• Collaborates with clinical team to finalize Integrated Service Plans and notes.

Job description

High Needs Case Manager: JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services.

Responsibilities

• Provides case management services to children enrolled in the behavioral-health system.
• Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
• Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
• Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
• Coordinates and facilitates Child & Family Team (CFT) Meetings.
• Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
• Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.

Qualifications

-Meets one of the following education/experience requirements:

• High School Diploma/GED required and 4 years of Behavioral Health experience.
• A.A. Degree and two years of Behavioral Health experience
• Bachelor’s Degree in Social Work, Psychology, or human services field.

• Level 1 Fingerprint Clearance Card without Restriction(s)
• Meet Agency requirements for MVR (Motor Vehicle Record) report
