Job summary

Jewish Family & Children’s Service of Arizona is seeking a High Needs Case Manager to support children in behavioral-health services toward independence and possible family reunification. The role includes guiding families and coordinating care across systems in the Valley.

You will work with a clinical team to ensure timely paperwork, plans, and notes while maintaining regular client contact and community involvement.

Qualifications

  • Meets one of the education/experience paths listed.
  • Behavioral health experience is a key requirement across paths.
  • Must meet agency requirements for background checks and driving eligibility.

Responsibilities

  • Provide case management for children in behavioral-health system.
  • Maintain weekly contact with clients through visits and checks.
  • Assist with discharge planning and access to intensive services as needed.
  • Coordinate services with DCS, Probation/Parole, DDD, schools, and providers.
  • Facilitate Child & Family Team (CFT) Meetings.
  • Process referrals, authorizations, and enrollment paperwork.
  • Collaborate with clinical team to complete Integrated Service Plans and notes.

Education

Bachelor’s Degree in Social Work, Psychology, or human services field.
A.A. Degree and two years of Behavioral Health experience
High School Diploma/GED with 4 years of Behavioral Health experience

Job description

Phoenix Location

High Needs Case Manager: JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services.

Responsibilities
  • Provides case management services to children enrolled in the behavioral-health system.
  • Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
  • Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
  • Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Coordinates and facilitates Child & Family Team (CFT) Meetings.
  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
Qualifications

-Meets one of the following education/experience requirements:

  • High School Diploma/GED required and 4 years of Behavioral Health experience.
  • A.A. Degree and two years of Behavioral Health experience
  • Bachelor’s Degree in Social Work, Psychology, or human services field.

-Additional qualifications:

  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • Meet Agency requirements for MVR (Motor Vehicle Record) report
  • Reliable transportation
