StephenCenter has served homeless and low-income individuals in Omaha since1984 with the mission to partner with the community, families andindividuals to overcome homelessness, addiction and poverty. ThePettigrew Emergency Shelter and Permanent Supportive Housing offeringsprovide safe and secure housing while individuals and families completean assessment of permanent housing options. All Stephen Center residentshave access to dual diagnosis support services via the HEROaddiction/recovery program. 100% of Stephen Center shelter supportivehousing participants are living in poverty. Housing transition supportidentifies poverty barriers that may impact one’s ability to achieve ormaintain permanent housing through case management, life skills, andbehavioral health services. Stephen Center serves approximately 250people daily in all programs.
Purpose:
The HERO CaseManager plays a critical role in supporting a coordinated, client-centeredrecovery environment by integrating clinical, operational, and community-basedservices. Working collaboratively across interdisciplinary teams, this positionensures safety, accountability, and continuity of care for individualsnavigating co-occurring substance use and mental health recovery. The rolefocuses on connecting clients to appropriate referrals and resources thatsupport both immediate needs and long-term recovery, while partnering withclinical staff, leadership, and administrative teams to maintain a safe andstructured program environment.
Through ongoing assessment, the Case Manager identifies client needsrelated to physical and mental health, housing, employment, family support, andother essential services, and works closely with the HERO care team andcommunity partners to implement individualized solutions. This positionsupports clients in preparing for independent living while ensuring allservices, client engagement, and outcomes are accurately and timely documentedin alignment with program standards, compliance requirements, and overallquality of care.
Key Accountabilities:
- Maintain a safe, structured, andtherapeutic environment by consistently following all safety, security, and program guidelines, including class participation, behavior compliance, andclient activity as needed.
- Facilitate and coordinate dailyrecovery programming, including groups, meetings, and structured activitiesaligned with the HERO continuum of care (STR, IOP, OP).
- Complete comprehensive intakes andclient orientations, ensuring everyone is welcomed into the program with clearexpectations, resources, and support.
- Develop individualized service planswithin 24 hours of admission, in collaboration with clinical staff andleadership, ensuring plans are person-centered, goal-oriented, and responsiveto client needs.
- Implement and manage service plansthrough ongoing engagement with clients, interdisciplinary teams, and communityproviders; documenting services, progress, barriers, and outcomes in acompliant manner.
- Ensure all documentation is enteredinto the Electronic Health Record within 72 hours of service delivery,maintaining accuracy, compliance, and alignment with agency and regulatorystandards.
- Coordinate access to medical, dental,psychiatric, and community-based services, strengthening partnerships tosupport whole-person care and successful transitions.
- Support high-acuity and dual-diagnosisclients coordination of care to reduce attrition and enhance retention.
- Assist clients in navigating systemsand completing applications related to healthcare, income, housing, and othersupports necessary for successful community reintegration.
- Participate actively ininterdisciplinary team meetings, contributing to case consultations, careplanning, and shared problem-solving to ensure continuity and quality of care.
- Support program operations throughdaily responsibilities including room checks, chore verification, medicationobservation, and reinforcement of program expectations and clientaccountability.
- Demonstrate strong communication,teamwork, and interpersonal skills, contributing to a positive, professional,and solution-focused team culture.
- Demonstratesstrong organizational and time management skills to effectively manage multipleresponsibilities in a fast-paced environment, balancing immediate client needswith long-term goals and outcomes .
- Uphold strict confidentiality andethical standards in all aspects of client care and program operations.
- Providesupport to program leadership as needed and contribute to ongoing programdevelopment excellence.
- Ensure all safety and security guidelines are followed.
- Coordination of all Recovery activities and meetings.
- Proficiency with Microsoft Office and excellent written, oral andinterpersonal skills.
- Other duties as assigned.
Position Requirements:
- Bachelor’s degree in social work, humanservices, psychology, or a related field; and/or equivalent experience .
- Experience working with individuals experiencinghomelessness, addiction, or mental health challenges preferred .

Salary: $19-$23 per hour
