Job description

Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.

Purpose:

The Street Outreach Coordinator plays apivotal role in Stephen Center’s mission to provide support, resources, andpathways to stability for individuals experiencing homelessness. Thisposition combines field-based outreach with program coordination, ensuringthat unsheltered individuals, shelter residents, and community partnersexperience consistent, compassionate, and effective services.

In addition to direct outreach and casemanagement, the Street Outreach Coordinator is responsible for fosteringcommunity partnerships, coordinating across Continuum of Care (CoC) teams,and contributing to the creation and implementation of policies andprocedures that ensure safe, trauma-informed, and effective service deliveryin unpredictable environments.

Key Accountabilities:

• Conduct proactive outreach to individuals andfamilies in encampments, public spaces, and other unsheltered areas inDouglas, Sarpy, and Pottawattamie counties.
• Build trusting relationships with clients,assess immediate needs, and connect them to shelter, housing,healthcare, and other resources.
• Provide crisis intervention and de-escalationwhile ensuring personal and client safety.
• Develop individualized case management plansfor unsheltered individuals and shelter residents, including goalsetting and progress monitoring.
• Maintain accurate, timely documentation andenter service data into Clarity within 24 hours.

Program Coordination & Oversight

• Act as a key liaison with Threshold Continuumof Care (CoC) and other outreach providers to ensure coordinatedservices.
• Support Stephen Center shelter operations bybridging information between outreach efforts and shelter admissions.
• Assist with shelter client case managementwhen needed, ensuring continuity of care between unsheltered and shelteredpopulations.
• Ensure all policies, procedures, andcompliance standards (including CARF and funder requirements) arefollowed.

Community Partnerships & Representation

• Develop and strengthen relationships withcommunity partners, law enforcement, healthcare providers, and housingagencies.
• Represent Stephen Center at communitymeetings, collaborative task forces, and training sessions.
• Advocate for trauma-informed, humaneapproaches to homelessness across all partnerships.

Policy,Procedure & Safety

• Collaborate with leadership to create,implement, and refine policies and procedures for outreach and sheltercoordination.
• Ensure consistency and clarity in safetyprotocols for staff and clients in unpredictable environments.
• Provide training, guidance, and feedback toteam members to strengthen field safety, professional boundaries, andeffective client engagement.

Qualifications

• Bachelor’s degree in social work, Psychology,Human Services, or related field (equivalent experience considered).
• At least 1 year of supervisory or programcoordination experience preferred.
• Demonstrated experience working withindividuals experiencing homelessness, mental health challenges, orsubstance use disorders.
• Strong interpersonal and crisis managementskills, with the ability to build rapport across diverse populations.
• Knowledge of Omaha-area community resourcesstrongly preferred.
• Valid driver’s license and reliabletransportation required.

WorkingConditions

• Combination of office, shelter, andfield-based work in varying environments, including outdoor encampmentsand public spaces.
• May involve exposure to challenging orunpredictable situations, requiring safety awareness and trauma-informedpractice.
• Evening or weekend hours will be required.
• Physical requirements: ability tosit, stand, walk, kneel, and lift up to 20 lbs.
