Job summary

Stephen Center in Omaha is hiring a HERO Case Manager to support a coordinated, client-centered recovery environment. You will work with interdisciplinary teams to ensure safety, accountability and continuity of care for clients navigating co-occurring substance use and mental health recovery.

Responsibilities include developing individualized service plans within 24 hours of admission, documenting services in the EHR within 72 hours, coordinating access to medical and housing resources, and

Qualifications

• Bachelor’s degree in social work, human services, psychology, or related field.
• Experience working with homelessness/addiction/mental health preferred.
• Standing/Walking/Kneeling Periodically; Lift up to 20 pounds Periodically.

Responsibilities

• Maintain a safe, structured, and therapeutic environment by following safety, security, and program guidelines.
• Coordinate daily recovery programming and activities aligned with HERO continuum of care.
• Complete comprehensive intakes and client orientations with clear expectations and support.
• Develop individualized service plans within 24 hours of admission with clinical leadership.
• Implement and manage service plans with documentation of progress, barriers, and outcomes in the EHR.
• Ensure all documentation is entered into the EHR within 72 hours of service delivery.
• Coordinate access to medical, dental, psychiatric, and community-based services.
• Support high-acuity and dual-diagnosis clients and coordinate care.
