Job description

StephenCenter has served homeless and low-income individuals in Omaha since1984 with the mission to partner with the community, families andindividuals to overcome homelessness, addiction and poverty. ThePettigrew Emergency Shelter and Permanent Supportive Housing offeringsprovide safe and secure housing while individuals and families completean assessment of permanent housing options. All Stephen Center residentshave access to dual diagnosis support services via the HEROaddiction/recovery program. 100% of Stephen Center shelter supportivehousing participants are living in poverty. Housing transition supportidentifies poverty barriers that may impact one’s ability to achieve ormaintain permanent housing through case management, life skills, andbehavioral health services. Stephen Center serves approximately 250people daily in all programs.

Purpose:

The Licensed SocialWorker serves a critical role within the HERO Program, providing comprehensiveinpatient and outpatient behavioral health treatment services to individualswith co-occurring substance use and mental health disorders. This position isresponsible for conducting clinical assessments, developing and implementingindividualized treatment plans, facilitating individual and grouppsychotherapy, and delivering both clinical and psychoeducational programmingon a daily basis. The therapist supports continuity of care across all levelsof programming (STR, IOP, and OP) while maintaining accurate and timelyclinical documentation in accordance with regulatory standards.

Purpose:

The Licensed Social Worker is expected to function within a structured,team-based environment, actively engaging in interdisciplinary collaborationand balancing clinical judgment with program expectations. Services must alignwith evidence-based practices, CARF standards, and all applicable federal,state, and grant requirements. This role is a key contributor to programculture, client engagement, and overall treatment outcomes, with clearexpectations for consistency, accountability, and professionalism inmaintaining a safe, therapeutic, and recovery-oriented environment. Additionalduties may be assigned to support program operations and client care.

K ey Accountabilities:

Service Delivery & Clinical Responsibilities

Providecomprehensive, evidence-based behavioral health services to individualswith co-occurring mental health and substance use disorders.
Conducttimely and accurate mental health and substance use assessments inaccordance with program and regulatory standards.
Develop,implement, and regularly update individualized treatment plans withmeasurable short- and long-term goals in collaboration with theinterdisciplinary team.
Facilitateindividual, group, and family counseling sessions, including dailyclinical and psychoeducational groups.
Conductsubstance abuse assessments, drug and alcohol evaluations, and chemicaldependency evaluations in accordance with clinical, state, and programrequirements, within the scope permitted by licensure.
Maintaina mixed caseload while ensuring continuity of care across all programlevels (STR, IOP, OP, Aftercare).
Continuouslyassess client progress and adjust treatment interventions based onclinical need and team collaboration.
Provideonsite crisis intervention and participate in an on-call rotation forafter-hours client emergencies.
Integrateculturally responsive and trauma-informed approaches into all aspects ofservice delivery.
Buildand maintain professional relationships with referral sources andcommunity partners to support coordinated care and program growth.
Promoteagency services through appropriate outreach, workshops, and communityengagement opportunities.

Adhere to all agency policies,procedures, and clinical protocols while operating within scope ofpractice and ethical guidelines.

Documentation, Compliance & Accountability

Maintaintimely, accurate, and compliant clinical documentation, includingassessments, treatment plans, progress notes, and discharge summaries.
Ensureall documentation meets agency, state, federal, payer, and CARFaccreditation standards.
Accuratelydocument client progress, participation, and outcomes in a clear,objective, and professional manner.
Collaboratewith finance and administrative teams to ensure services are authorized,billable, properly coded, and submitted in a timely manner.
Completeprovider enrollment, reporting requirements, and required documentation tosupport reimbursement and compliance.
Maintainrequired records for courts, external systems, and regulatory entities asapplicable.

Document and participate in clinical supervision in alignment withlicensure requirements, when applicable.

PositionRequirements:

Hold licensurein the State of Nebraska as a PLADC, LADC, PLMHP, LIMHP, or LicensedSocial Worker; provisional licensure at minimum is required.
Activelypursue and maintain appropriate licensure, supervision, and continuingeducation requirements in alignment with state regulations and scope ofpractice.
Demonstratethe ability to assess, diagnose, and develop treatment recommendations forindividuals with co-occurring mental health and substance use disorders,within the scope of licensure.
