Connectly Recruiting is seeking a Cardiologist (Heart Failure Specialist) for full-time, permanent placement in Indiana. The role offers a target base salary of $550,000–$650,000+ with wRVU incentives, sign-on negotiable, and relocation options.

Inpatient and outpatient heart failure care, leadership of HF team, and collaboration with multidisciplinary teams are key aspects. Flexible operating hours and no general cardiology call are noted, with comprehensive benefits including insurance, 403(b)

Qualifications

• MD or DO with internal medicine residency and cardiology fellowship.
• Fellowship in Advanced Heart Failure & Transplant Cardiology.
• Board Certification in Cardiovascular Disease; BC/BE in Advanced Heart Failure.

Responsabilités

• Provide comprehensive heart failure management across inpatient consult services and outpatient heart failure clinic settings.
• Diagnose and treat complex cardiomyopathies and advanced heart failure using GDMT and modern clinical protocols.
• Manage outpatient IV diuretic administration and utilize remote hemodynamic monitoring technologies to adjust treatment plans.
• Perform diagnostic workups including echocardiography, nuclear imaging, and diagnostic right heart catheterizations.
• Provide physician oversight and guidance for Heart Failure Advanced Practice Providers within a collaborative practice model.
• Partner with Structural Heart teams, Electrophysiology, Cardiac Rehab, Palliative Care, Dietitians, and Social Work.
• Lead hospital readmission reduction, GDMT optimization, clinical pathway implementation, and quality metric tracking.
• Collaborate with general cardiologists, PCPs, and hospitalists to streamline referral care pathways.

Formation

MD or DO with ACGME-accredited Internal Medicine residency and Cardiovascular Disease fellowship

Fellowship training or specialized clinical focus in Advanced Heart Failure & Transplant Cardiology

Board Certified in Cardiovascular Disease; BC/BE in Advanced Heart Failure
