Job summary

Enhabit Home Health & Hospice seeks a dynamic professional to manage relationships with referral sources and promote post-acute care services. You will educate providers, navigate care transitions, and grow partnerships to deliver high-quality patient outcomes.

Requires a healthcare-related bachelor's degree and experience in business development or sales within the home health/hospice sector. Strong communication, organization, and driving is essential for success.

Qualifications

• Bachelor's degree or equivalent in healthcare/related field.
• Two years of healthcare business development, community relations, marketing, or sales experience.
• Knowledge of Medicare/home health regulatory guidelines.

Responsibilities

• Serve as primary relationship manager with referral sources (physicians, hospitals, health systems).
• Educate referral sources about Enhabit services to grow referrals and improve patient outcomes during post-acute transitions.
• Maintain and expand relationships with physicians, hospitals and senior living communities.
