Job summary

Whitehall Borough Post Acute is a 166-bed post-acute care facility seeking a proactive, relationship-driven professional to develop and maintain referral partnerships with hospitals, social workers, physicians, and community providers.

The role emphasizes hospital visits, referrals management, and collaboration with the Director of Business Development to drive occupancy while supporting marketing initiatives and outreach efforts across the PACS network.

Qualifications

• Ability to maintain continuing education requirements.
• Must be 18 years or older.
• Clear background check.
• Medical physicals and PPD as required.

Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, physicians, and referral partners.
• Conduct hospital and bedside visits and follow-ups to strengthen relationships and referral conversion.
• Review referrals and coordinate with clinical and admissions teams.
• Identify and develop new referral and community partnership opportunities.
• Represent the facility at vendor fairs, networking events, and outreach opportunities.
• Support marketing initiatives including social media, newsletters, and promotional materials.
