Job summary

Covenant Living of Turlock is seeking a Healthcare Navigator who will advance admissions for skilled nursing by cultivating referral partnerships and leading outreach to physicians, hospitals, and community organizations. You’ll utilize CRM tools to monitor leads and coordinate with the admissions team to optimize patient placement.

The role requires five years of related sales experience, Medicare knowledge, and the ability to travel up to 90% of the time, with a Bachelor’s degree or equivalent

Qualifications

• Bachelor’s degree or equivalent work experience.
• Five years related experience.
• Proven and effective sales experiences, from lead generation, education to clients, B2B sales strategies, follow up with leads and strong closing skills.
• Knowledge or understanding of Medicare (Part A & B), and managed care.
• Valid driver’s license with reliable transportation.
• Ability to travel within market up to 90% of the time.

Responsibilities

• Develop and maintain relationships with referral sources to drive admissions/move-ins for skilled nursing.
• Lead outreach visits to referral sources and influencers to meet sales goals.
• Present CRC products and services to groups and individuals; conduct one-to-one presentations.
• Analyze market trends and adjust census development plans in collaboration with campus stakeholders.
• Manage CRM data, track outcomes, and report monthly results to leadership.
• Assist with admissions decisions and coordination with clinical and admin staff.
• Maintain confidentiality and provide excellent customer service to residents and families.
