Job summary

Hospice of the Midwest in Forest Lake, MN is seeking a proactive Hospice Team Coordinator to support care teams and clinical leadership. This role emphasizes organization, attention to detail, and compassionate communication to ensure smooth scheduling, documentation, and patient support.

Hours are Monday–Friday, 8:00 AM–5:00 PM. You will maintain patient records, coordinate admissions and discharges, manage equipment and supplies, and assist HR and compliance functions to support high-quality

Qualifications

• High School Diploma or GED required.
• Minimum of two years of administrative healthcare work experience.
• Proficient in verbal and written communication with patients, families, and staff.

Responsibilities

• Provide administrative support to clinical leadership and care teams.
• Maintain organized patient records, office files, and supplies.
• Answer phones, manage mail, and assist visitors with professionalism.
• Track and order medical equipment and office supplies.
• Input data into electronic systems (e.g., HCHB) and set up clinician tablets.
• Assist with scheduling, admissions, discharge, and team coordination.
• Support HR functions like maintaining employee records.
• Ensure compliance with hospice policies and Medicare regulations.
