Job summary

Hospice of the Midwest in Forest Lake, MN is seeking a Hospice Team Coordinator to provide essential administrative support to clinical leadership and care teams. You will maintain patient records, manage office supplies, answer phones, and assist with scheduling, admissions and team coordination.

Ideal candidates have at least two years in healthcare administration, familiarity with HCHB, and solid communication skills.

Qualifications

• High School Diploma or GED required.
• Minimum of two years of administrative healthcare work experience, preferably in hospice and/or home health operations.
• Ability to maintain orderly files, supplies, and office systems.
• Proficient in verbal and written communication with patients, families, and staff.
• Familiarity with data entry systems such as HCHB and basic office software.
• Understanding of Medicare documentation standards and referral/intake

Responsibilities

• Track and order medical equipment and office supplies.
