Job summary

West Somerset Primary Care Network is seeking a Health and Wellbeing Coach to work across practices and community locations. The role focuses on patient-centered coaching, addressing long-term conditions, prevention, motivation, and wellbeing through face-to-face, telephone, and digital appointments.

You will use active listening and evidence-based coaching tools, prioritise referrals by risk, and collaborate with social prescribing and clinical colleagues to support patients' health priorities.

Qualifications

• Structured, person-centred coaching through various formats.
• Use evidence-informed behaviour change techniques.
• Prioritise referrals based on patient need and risk.
• Coordinate with social prescribing and clinical colleagues.

Responsibilities

• Deliver a regular programme of coaching appointments and reviews.
• Manage an active caseload with group sessions when appropriate.
• Respond to referrals through PCN pathways and assess suitability.
• Maintain knowledge of local community services to connect patients.
