Job summary

Volunteers of America National Services (VOANS) is seeking a Health Unit Coordinator‑LPN at Rochester Rehab & Living Center in Rochester, MN. In this vital role, you will provide essential, compassionate care to residents, assisting them with daily living activities while promoting their well-being.

Qualifications

• Must hold a current LPN and/or Health Unit licensure in Minnesota.
• Experience in long-term care is preferred.
• Excellent communication and customer service skills are essential.

Responsibilities

• Provide direct hands-on care, including assistance with daily living activities.
• Support residents’ physical and emotional well-being.
• Monitor changes in residents’ conditions and report to nursing team.
• Maintain a clean, safe environment while following care plans.

Pay Range: $34.00 - $39.00

Schedule: 10:00am‑6:30pm or 11:00am‑7:30pm M‑F

What You Bring (Requirements)

• Current LPN and or Health Unit licensure in MN
• Strong communication and customer service skills
• Ability to follow care plans and resident orders
• Long‑term care experience (preferred)

What You’ll Be Responsible For

• Provide direct, hands‑on care to residents, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility
• Support residents’ physical and emotional well‑being by building meaningful connections and delivering compassionate, person‑centered care
• Monitor and report changes in residents’ condition to the nursing team to ensure timely and appropriate care
• Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards
