Job summary

CareStar, Inc. in Cincinnati, OH seeks a Rising Star to join our Support Services team. You will assess needs, develop personalized care plans, and connect clients with home-based health services and essential supports.

Qualifications

• Work a minimum of 20 hours per week.
• One-year commitment.

• Proficient with Microsoft Office programs.

Responsibilities

• Rising Stars will work directly with our community's most vulnerable to assist them obtaining community based in-home health services.
• Rising Stars gain leadership experience by participating in, and occasionally leading, cross-training efforts to ensure adequate coverage for all support services tasks and departmental projects.
• Rising Stars will be responsible for providing coordination of the Support Services Pool by utilizing in-depth knowledge of company functions to effectively schedule assessments and provide support services to internal and external clients.
