Job summary

Source One Management Services, LLC in Olive Hill, KY is seeking Nurse Practitioners for a flexible PRN in-home Health Risk Assessment (HRA) role for Medicare Advantage members. This position lets you set your own schedule and supplement income without ongoing treatment responsibilities.

In this role you will complete comprehensive HRAs, review medical histories, medications, lifestyle and social factors, and perform basic physical and mental health evaluations.

Qualifications

• Active NP license (excluding Pediatrics, PMHNP and Women's Health specialties).
• Minimum availability of eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of your home.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review each patient's medical history, current medications, lifestyle, and key social factors.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
