Job summary

Source One Management Services, LLC is seeking Nurse Practitioners for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This PRN role lets you work autonomously while making a real difference in patient care, with no on-call shifts and no ongoing treatment.

Responsibilities include completing comprehensive in-home HRAs, reviewing medical histories, medications, lifestyle and social factors, and conducting basic physical and mental health evaluations.

Qualifications

• Active NP license (excluding Pediatrics, PMHNP and Women’s Health specialties).
• Minimum availability of eight (8) full 7.5‑hour days per month.
• Willingness to travel within roughly 60 minutes of home.
