Job summary

Source One Management Services, LLC is seeking Nurse Practitioners for a flexible PRN in-home Health Risk Assessments for Medicare Advantage members. This position lets you set your schedule with an 8-day monthly minimum and work autonomously.

You will complete HRAs, review medical histories, medications, lifestyle and social factors, conduct basic physical and mental health evaluations, and travel locally within about 60 minutes of your home.

Qualifications

• Active NP license is required.
• Minimum availability of eight (8) full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.
