Job summary

Hearts With A Mission in Jackson County is seeking a full-time Case Manager to collaborate with Safe Families for Children and Hearts For Seniors. The role focuses on assessing client needs, developing Individual Service Plans, and building strong relationships with schools, service providers, and community partners.

The position requires a Bachelor’s degree and at least two years of experience with at-risk populations, plus a valid Oregon driver’s license.

Qualifications

• Bachelor’s Degree in Education, Social Work, Psychology or related field; equivalent combination considered.
• Minimum of two years’ experience working with at-risk clients.
• Valid Oregon driver’s license at time of hire.

Responsibilities

• Recruit volunteers and coordinate recruitment strategies.
• Perform initial intake screening and help develop Individual Service Plans.
• Maintain relationships with schools and community partners.
• Document client interactions and manage data/reporting.
• Ensure confidentiality and compliance with laws and agency guidelines.

Education

Bachelor’s Degree in Education, Social Work, Psychology
